Our Practice accepts most insurance plans in our area including Medicare, Medicaid, HMO's, PPO and indemnity insurance plans. Please Check with your insurance company to verify that we are providers for your plan. If you are part of an HMO or other policy that requires referrals to our office, please coordinate with your primary care physician for a referral prior to your appointment with us. Enclosed with the New Patient Packet is a Lab Request Form. Please have your lab done at the lab of your choice a day or two before your appointment. You can check with your insurance company regarding participating laboratories with your insurance plan. If you have had lab work recently, please call to let us know. It is important for our physicians to have these results so a proper diagnosis can be determined.
If at all possible, we will send you a New Patient Packet along with a Lab Request Form when we make your first appointment. We ask that you complete the Packet prior to your first appointment and bring it with you along with your driver's license and insurance card(s). Please arrive 15 minutes prior to your appointment time to allow the front desk staff to copy those documents. In addition, we ask that you have your lab done prior to your appointment to allow the physician to have the results available at the time of your visit.
Please bring all medications that you are currently taking. In addition, please bring your current insurance card and driver's license to each visit. We ask that you arrive 15 minutes prior to your appointment time so that we can update your insurance records or make any changes necessary to your personal information. We also want you to know that we verify insurance benefits and determine your financial obligation in advance. Please come prepared to pay your applicable co-pay and/or deductible at the designated appointment time.
Payment for professional services is due at the time of service for non-insured patients.
We honor most major credit cards, as well as accept cash or personal check.
If insurance is to be filed, you are still responsible for charges that are not covered by your insurance plan. If you need to make payment arrangements, please contact the billing staff at 318-220-9788.
